Job description
Join a national provider delivering in-home health assessments to improve patient outcomes and care quality. As a provider, you will conduct comprehensive assessments lasting up to one hour, reviewing medical history, medication adherence, social factors, and health risks. Responsibilities include performing adult/geriatric evaluations, administering tests, collaborating with healthcare teams, and providing education and follow-up at members’ residences, including homes and facilities.
Qualifications include a Master’s Degree or equivalent experience, current NP/PA licensure with prescriptive authority, board certification in relevant specialties, and at least one year of experience. Strong communication skills, computer proficiency, and the ability to travel extensively are essential. A valid driver’s license and appropriate insurance coverage are required.
