Nurse Practitioner or Physician Assistant, House Calls WA Statewide Traveler
$40,000 Sign-on Bonus or $40,000 Student Loan Repayment

About the Role As a HouseCalls Advanced Practice Clinician (NP or PA), you'll perform annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, field-based role focused on preventive care, patient education, closing care gaps, and helping members access the resources they need to improve their health.
Key Responsibilities
Conduct comprehensive in-home health assessments
Review medical history, medications, vital signs, and perform physical exams
Identify care gaps and communicate findings to PCPs
Educate patients on health conditions and treatment plans
Address social determinants of health and connect members with community resources
Recognize urgent health concerns and facilitate appropriate follow-up care
Qualifications
NP: Active AANP or ANCC certification
PA: Active NCCPA certification
Active, unrestricted licensure (or ability to obtain)
Valid driver's license and reliable transportation
Ability to travel extensively throughout Washington
Preferred
1+ year of clinical experience
Experience with geriatric, primary care, home health, or Medicare populations
Strong communication and technology skills

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
