nThis is a per diem position where you get to choose your hours and must be available at least 24 hours per month.

nThe Senior Community Care (SCC) program is an integrated care delivery program that coordinates the delivery and provision of clinical care of members. The Advanced Practice Clinician within SCC provides care to our highest-risk health plan members. nPrimary Responsibilities:
nnnConduct comprehensive assessments, including:
n
nnPast medical history review, medication reconciliation, vital signs, and physical exam
n
nnEvidence-based screenings and point-of-care testing (as appropriate)
n
nnSupport members during a short term sub-acute rehabilitation episode
n
nnEnsure accurate and complete documentation, including ICD 10 conditions
n
nnCommunicate and collaborate with the interdisciplinary care team
n
nnConduct advanced illness and advanced care planning conversations
n
nnProvide patients and caregivers with counseling and education
n

nRequired Qualifications:
nnnFor NPs: Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
n
nnFor PAs: Current NCCPA certification and state licensure
n
nnActive, unrestricted licensure in the state you will be practicing in (or ability to obtain by start date) without current disciplinary actions or practice limitations
n
nnActive and unrestricted driver's license
n
nnIn addition to a driver's license, access to reliable transportation
n
nnCurrent, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
n
nnAbility to gain a collaborative practice agreement, if applicable in your state
n
nnAbility to navigate varied environments and to position oneself as needed to perform job duties
n
n nPreferred Qualifications:
nnn1+ years of clinical experience as an APC (long-term care setting preferred)
n
nnExperience in geriatric medicine, long-term care, senior living or home care setting
n
nnExperience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
n
nnUnderstanding of Geriatrics and Chronic Illness
n
nnUnderstanding of Advanced Illness and end of life discussions
n
nnProficiency with electronic medical records and technology
n
n nCompensation for this specialty generally ranges from $53.00 to $79.00 per hour. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable.
