Job description
Role Overview: This position involves conducting comprehensive in-home health assessments for adult and geriatric members, lasting up to one hour. Providers evaluate medical history, current health, medications, social environment, and risks to improve care quality and close care gaps.
Responsibilities
Perform assessments, including medical history, diagnosis, treatment, health education, and referrals.
Administer and order point-of-care and lab tests.
Collaborate with physicians, case managers, social workers, caregivers, and other medical staff.
Coordinate follow-up and patient education with primary care providers.
Conduct services at members’ residences, including homes and skilled nursing facilities.
Qualifications & Skills
Master’s Degree or equivalent experience with NP or PA licensure.
Current licensure in practice state, including prescriptive authority.
Board certification in adult or geriatric primary or acute care.
CPR/BLS/ACLS certification.
Preferred: 1+ year clinical experience; new grads encouraged.
Valid driver’s license, reliable vehicle, computer skills, and strong communication abilities.
