About the Role
nHealthonomic Primary Care is hiring a full-time Nurse Practitioner for our Orland Park and Palos Heights offices. You'll care for an established adult panel, with an emphasis on chronic disease management and preventive care, working closely with our physician team.
nResponsibilities
nProvide primary care to adult patients, including chronic disease management
nPerform Medicare Annual Wellness Visits and preventive care visits
nSupport our chronic care management program, including patient care plans and follow-up
nOrder and interpret labs and imaging; manage medications and referrals
nDocument care in Epic and support quality and preventive care initiatives
nRequerments
nActive Illinois APRN/Nurse Practitioner license in good standing
nFNP or AGNP national certification (ANCC or AANP)
nActive DEA and Illinois controlled-substance registration, or eligible to obtain before start
nCurrent BLS certification
n2+ years of nurse practitioner experience in primary care, family practice, or internal medicine
nChronic disease management experience with adult patients
nComfortable documenting in an EHR; Epic experience preferred
nAble to work across two offices weekly — Orland Park Monday–Tuesday, Palos Heights Wednesday–Friday
nPreferred: Chronic Care Management (CCM) or care-coordination experience, Medicare Annual Wellness Visits, quality/HEDIS familiarity.
