Job description
MVHC in Zanesville, OH is seeking a Nurse Practitioner to join the Transition of Care/Primary Care team. This 100% outpatient role offers collaboration with physicians, social workers, RNs, and LPNs in a supportive environment.
The NP will conduct post‑discharge visits, manage chronic diseases, perform medication reconciliations, and coordinate comprehensive care plans with emphasis on social determinants of health. This full‑time position includes benefits and no on‑call obligations.
