Nurse Practitioner - Palliative Care

nHospice de la Luzis seeking aPRN (per diem)dedicated and professionalNurse Practitionerto join our team serving our patients currently enrolled in ourpalliative care servicesin theAlbuquerqueand surrounding areas doing face to face visits.

nPrimary functions are to assist in overseeing the clinical care and treatment of patients and families; to consult with the patients’ attending physician, in consultation with the hospice physician; provide clinical direction to the Interdisciplinary group to ensure quality care.
nRequirements
nJob Qualifications:
nnnEducation: Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
n
nnLicensure: Current State license as a Registered Nurse and as an Advanced Practice Nurse. Current Driver’s License.
n
nnExperience: Two years' experience as an APN, clinical care setting preferred; hospice or home health preferred.
n
nnSkills: Nursing skills as defined as generally accepted standards of practice. Comprehensive knowledge of pharmacology, palliative care, and end-of-life. Good interpersonal skills.
n
nnTransportation: Reliable transportation and valid auto liability insurance.
n
nnOn-call requirement 1-2 weeks a month
n
n nEssential Functions:
nnnCollaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease.
n
nnConsults with and assists in the IDG’s development of patient/family plan of care to assure quality care is provided.
n
nnConsults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
n
nnPerforms and documents visits to patient/family based on plan of care and patient/family need.
n
nnInteracts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician.
n
nnConsults with and provides education to other IDG members on a prn basis for clinical issues relating to palliation.
n
nnActively participates in the IDG. Communicates effectively, timely, and appropriately at all times.
n
nnInforms physicians and other personnel of changes in the patient’s needs and outcomes of intervention, while evaluating patient/family response to care.
n
n nEnvironmental and Working Conditions:
nWorks in patients’ homes in various conditions; possible exposure to blood and bodily fluids and infectious diseases; the
nability to work a flexible schedule; the ability to travel locally; some exposure to unpleasant weather; PRN emergency call.
nBenefits:
nnnFace to face per visit rate - $125.00 - $150.00
n
nnAdditional monthly stipend paid for on-call, attending IDG meetings, documentation etc.

nnMileage reimbursement
