nWhat You Need to Know

nBe the Best Palliative Care Nurse Practitioner You Can Be  

nIf you meet these qualifications, we would love to meet you: 

nnnGraduated from an approved school of professional nursing

n

nnOne year experience as a Nurse Practitioner is preferred

n

nnLicensed to practice as a Nurse Practitioner in the state of agency operation

n

nnActive DEA license in the state where care is being provided.

n

nnPossess and maintain valid CPR certification while employed in a clinical role, preferred

n

nnMust be a licensed driver who can travel to all business locations

n

nnMeet the regulations and requirements of the state(s) in which program provides services

n

n nResponsibilities:   

nAs a Palliative Care Nurse Practitioner, you will:

nnnDevelop and implement clinical plans of care for patients with chronic and terminal illnesses

n

nnPerform patient assessment, history and physical; recommendations on aggressive symptom-management

n

nnMonitor patient progress; update the Medical Director, Team Director and team members; consult regarding discharge plannning; and patient/family teaching

n

nnMaintain communication with the supervisory physician (s) and attending physician to review patient records and care plans on a routine basis

n

nnInitiate communication with attending physicians, other Palliative Care staff members and other agencies as needed to coordinate optimal care and use of resources for patient/family

n

nnMay provide support at time of death

n

nnPrescribe medication as permitted by the state and/or the State Board of Nursing

n

nnParticipate in continum of care for patients that are hospice appropriate and ongoing education programs.
