You’ll be providing exceptional care to patients in a post-acute care setting, utilizing your expertise in family medicine or internal medicine. Don’t worry if you’re fresh out of school—we’re excited to welcome new grads who are ready to learn, grow, and make a difference.
What You’ll Do:
Deliver top-notch care to post-acute patients Monday through Friday (weekends are for you!).
Manage patient assessments, care plans, and follow-ups.
Collaborate with a supportive interdisciplinary team that believes in quality care (and quality coffee).
Document care with precision because every patient deserves a note-worthy experience.
What You Bring:
Certification as a Nurse Practitioner (CNP, FNP, APRN) or Physician Assistant (PA).
A passion for family or internal medicine.
The ability to thrive in a dynamic, post-acute care environment.
