Job Title
Nurse Practitioner / Physician Assistant - Physical Medicine & Rehabilitation
Company Overview
Total Extended Care Services is a growing Physical Medicine and Rehabilitation (PM&R) group focused on helping patients regain function, improve quality of life, and reduce avoidable hospital readmissions in skilled nursing and sub-acute rehabilitation settings. Our clinicians work closely with interdisciplinary care teams to deliver consistent, patient-centered rehabilitation care in senior living communities.

This role offers the opportunity to work alongside Board-Certified PM&R physicians in a specialized rehabilitation environment where your clinical judgment and collaboration directly impact patient recovery outcomes. Maintain timely and accurate documentation while balancing efficient patient care delivery
Contribute to a patient-centered, team-oriented culture focused on measurable rehabilitation outcomes and patient satisfaction
