Job description
Join a mission-driven health organization to deliver high-quality, accessible primary care to a diverse community.
Provide direct patient care, emphasizing health promotion, illness prevention, and continuity of care.
Conduct comprehensive histories, physical exams, and order appropriate tests.
Develop and review clinical protocols, ensure thorough documentation, and adhere to clinical guidelines.
Participate in quality improvement, team collaboration, and support the Patient Centered Medical Home model.
Supervise students and staff, contribute to community health initiatives, and maintain compliance with regulations.
Qualifications include NY licensure as a Nurse Practitioner or Physician Assistant, with experience in primary care. Bilingual abilities and community health experience are preferred.
Salary ranges from $100,000 to $140,000 annually. All qualified applicants are encouraged to apply regardless of background.
