About the Position:

The Team Coverage (TC) Provider plays an important role in maintaining continuity, access, and timely care within the Primary Care setting. When an empaneled primary care provider is unavailable, the Team Coverage Provider provides coverage for their customer-owners and supports by addressing both acute and ongoing primary care needs.

Team Coverage Providers care for customer-owners across the lifespan, from newborns through end of life. Responsibilities may include acute and same-day visits, preventive care, well-child checks, chronic disease management, emergency department follow-up, hospital discharges, inbox management, and supporting population-based health measures and other care team priorities.

The Team Coverage Provider role is primarily focused on bridging gaps in access and providing timely, comprehensive primary care when the empaneled provider is unavailable. The position requires adaptability and flexibility, as coverage needs may vary based on provider availability, customer-owner needs, and clinic priorities.

As a Team Coverage Provider, you can expect:

A supportive and collaborative team environment

Accessible clinical and organizational resources

No on-call

Opportunities for professional growth and development

A dedicated home clinic and strong connection to the care team

Variety in customer care and clinical experiences

The opportunity to make a meaningful impact by improving access and continuity of care

The Team Coverage Provider role offers the flexibility to meet evolving clinic needs while providing a strong foundation for professional growth, collaboration, and meaningful primary care delivery.
