Job description
Upperline Health is seeking a Nurse Practitioner to provide clinic-based care in our Padonia, MD clinic. In collaboration with the local Physician team, APPs develop complex care plans and provide diagnostics and treatments in clinic, home, or telehealth, aligned with scope of practice and patient goals.
The ideal candidate will have primary care/internal medicine experience, geriatric or home-based care exposure, and a passion for value-based care.
