Job DescriptionJob DescriptionThe Nurse Practitioner in Primary Care plays a vital role in delivering comprehensive healthcare by assessing, diagnosing, and managing both acute and chronic conditions. This position involves providing patient-centered care with a typical patient load of 15-25 patients per day, emphasizing preventive care, treatment planning, and effective medication management. The role demands strong care coordination and patient counseling skills to promote overall health and well-being.
Responsibilities
Conduct patient assessments and physical examinations
Diagnose illnesses and develop treatment plans
Manage medication plans and adjustments
Provide health education and preventive care guidance
Manage chronic diseases with ongoing monitoring
Coordinate care with multidisciplinary teams
Counsel patients on lifestyle and health decisions
Maintain accurate electronic medical records
Preferred Qualifications
Minimum 1 year of experience in primary care nursing
Master's degree in Nursing or related field
Active Nurse Practitioner License
Strong clinical assessment and pharmacology knowledge
Excellent patient communication and counseling skills
Experience with chronic disease management and health promotion
Proficient with Electronic Health Records (EHR) systems
Critical thinking, empathy, and effective decision-making abilities
