Job description
Oak Street Health is seeking a Nurse Practitioner to provide effective, value-based primary care to adults on Medicare in outpatient settings. You will work with a collaborative care team to deliver longitudinal care and build meaningful patient relationships, supporting care plans and population health initiatives.
