Job description
Oak Street Health in Winston-Salem, NC is seeking a Nurse Practitioner to provide effective, equitable value-based primary care to adults on Medicare, keeping patients healthy and out of the hospital. NPs work independently and with a physician-led team to deliver longitudinal care and strong patient relationships.
The role involves routine and acute visits, annual wellness exams, care coordination with specialists, and patient education.
