Job description
Transitions LifeCare is seeking a Nurse Practitioner to deliver comprehensive primary care in our Raleigh on-site clinic, following a Monday–Friday 8:30 am–5:00 pm schedule. You will work under guidance of the Chief Medical Officer and Medical Director, integrating care across inpatient, residential, home-based, and community settings to improve patient outcomes.
