nPOSITION OVERVIEW:

nThe Nurse Practitioner is responsible for working alongside our physicians and facilitating advanced primary and specialty nursing services to our patients. The primary responsibility will be to provide advanced primary and/or specialty nursing care to patients in the facility. Duties will include, but are not limited to, performing physicals exams, recording patient medical histories, creating patient care plans, administering treatment and diagnostic tests and observing the patient’s response to treatment.

nKEY ESSENTIAL FUNCTIONS:

nnnConduct psychiatric evaluations and assessments

n

nnConduct mental status examinations and pharmacological evaluations

n

nnPrescribe medications, teach clients about side effects and monitor response to prescribed medications, including ordering of laboratory tests as needed

n

nnAdminister injectable medications as needed

n

nnComplete all documentation (i.e. progress notes and psychiatric assessments) as required in a timely manner in the electronic medical record (EMR)

n

nnReview and provide recommendations for treatment plans

n

nnProvide expedited services to clients based on risk assessment status as clinically indicated.

n

nnProvide crisis intervention services to clients as needed

n

nnCommunicate with other medical practitioners in order to coordinate treatment for clients.

n

nnSpeak with managed care insurance representatives to obtain authorizations for prescribed medications when needed

n

nnParticipate in staff meetings and case conferences

n

nnCollaborate with primary therapists, clinical supervisor and program director to discuss status of client progress in treatment and provide input/guidance on treatment planning and risk management issues.

n

nnMaintain an 80% show rate for clients who are scheduled for regular appointments

n

nnProvide a behavioral risk screening of the newly admitted residents, and share the findings with the primary care team. Provide a comprehensive psychiatric evaluation as requested.

n

nnProvide ongoing psychopharmacological management as clinically appropriate.

n

nnRecommend and provide behavior management interventions as appropriate and functions as a member of behavior rapid response team.

n

nnCollaborate psychiatric management with the members of the primary care team and the attending psychiatrist.

n

nnParticipate in inter-departmental collaborative committees as needed and quality assurance and performance improvement activities.

n

nnDocument clinical activities in the medical record, and maintain documentation of administrative activities.

n

nnMaintain a collaborative relation with an attending psychiatrist for all clinical activities and seek supervision as needed.

n nREQUIREMENTS:

nnnActive and unrestricted New York State Nurse Practitioner license.

n

nnActive and unrestricted New York State Registered Nurse license.

n

nnActive and unrestricted NPI and DEA registration.

n

nnDATA 2000 Waiver.

n

nnMaster’s Degree in Nursing/Related field completion of Accredited NP Program required.

n

nnCurrent Infection Control certification.

n

nnCurrent Basic Life Support certification

n

nnThree years of practical experience preferred

n

nnAbility to work on a strong team of professionals in a culturally diverse environment.

n

nnAbility to multitask and to work and function under pressure.

n

nnBilingual – Spanish strongly preferred.

n

nnExperience or desire to work with people who have a mental illness

n

nnPositive attitude and professional demeanor

n

nnAbility to complete work independently as well as in collaboration with team members

n

nnMust be team oriented with a willingness to be flexible and helpful to move the organizational goals

n

nnExcellent computer skills including, Microsoft Windows, Excel, PowerPoint, and electronic communications tools: internet and email

n

nnAbility to communicate effectively orally and in writing

n

nnHighly organized, motivated self-starter to meet organizational goals

n

nnExcellent time management skills

n

nnMust be board certified by either ANCC or AANP.
