n nnAs a Nurse Practitioner You will:
nnnProvide compassionate medical care and symptom management
n
nnPerform assessments, order / interpret diagnostic tests, and develop care plans
n
nnPrescribe medications, manage treatments, and coordinate hospice services
n
nnConduct and document face-to-face visits for eligibility and recertification
n
nnCollaborate with physicians and IDT team to align care with patient goals.
n
nnParticipate in care planning and coordinate with team and external providers
n
nnMaintain accurate clinical documentation and ensure compliance regulation
n
nnEducate patients & families on medical, emotional, and spiritual needs
n
nnContribute to quality improvement efforts and professional development
n
nnServe as a clinical resource and liaison to community partners
n
n nResponsibilities
nnnFacilitate new patient consultation and health history review
n
nnPerform physical examinations and screenings of patients
n
nnOrder and administer tests including bloodwork
n
nnReview and interpret test results
n
nnDiagnose health problems
n
nnDetect changes in patient condition
n
nnAdminister medications
n
nnOperate medical equipment
n
nnRecommend further treatment options or future appointments if needed
n
nnProvide referrals to other practitioners when necessary
n
nnProvide after-care/ongoing care information and resources to patients
n
nnEnsure patient records are kept up to date including test results, immunizations, condition changes, etc.
n
nnDeliver quality, physician-level care while complying with company policies, safety regulations, and productivity metrics
n
nnAll other duties as assigned
n
n nQualifications
nEducation:
nnnMaster's degree in nursing
n
nnCurrent state nurse practitioner license required
n
nn5+ years nursing experience required
n
n nQualifications:
nnnStrong knowledge base of HIPAA and OSHA required
n
nnStrong interpersonal communication skills, both verbal and written
n
nnPatient, compassionate nature
n
nnAbility to operate as part of a team, alongside physicians and other medical staff
n
nnProficiency in medical instruments and equipment
n
nnProficiency in computer-based data management programs and information systems preferred
n
nnPrescriptive authorization required
n
n nWork Requirements:
nnnActive unrestricted license to practice in state
n
nnCertificate/license as a Nurse Practitioner to meet individual state practice act
n
nnCurrent DEA registration if applicable
n
