Nurse Practitioner

Compensation: $131,000 – $170,000 annually (based on experience) Relocation assistance available

Job Type: Full-time, Mondays to Fridays 8-5 PM or 4 day (10 hour shift available).

Location: Greater Sacramento area, CA (Multiple locations available)

We are seeking a full-time Advanced Practice Clinician (Nurse Practitioner or Physician Assistant) to provide comprehensive primary care services within a community health center setting. This role offers the opportunity to serve diverse and underserved populations while working collaboratively with physicians and multidisciplinary care teams.

Key Responsibilities

Perform comprehensive patient evaluations, including history and physical examinations

Diagnose and manage acute and chronic medical conditions

Order and interpret laboratory tests, imaging, and other diagnostics

Prescribe medications and therapeutic interventions within scope of practice

Develop and implement treatment plans in collaboration with the Medical Director

Monitor patient progress and modify care plans as appropriate

Initiate emergency procedures when clinically indicated

Provide preventive care, health education, and counseling services

Maintain accurate and timely documentation in accordance with agency and regulatory standards

Collaborate with physicians and other healthcare professionals to ensure coordinated, high-quality care

Qualifications

Active California RN and Nurse Practitioner license

Current Nurse Practitioner board certification

Current DEA certification

Current BLS certification

1–2 years of clinical experience preferred, particularly in community health or underserved populations

Strong commitment to serving low-income, homeless, or medically underserved communities

Comfortable treating three patients per hour

EPIC experience

In-basket management competency

Bilingual skills in English and Russian, Farsi, Pashto/Pushto, or Spanish strongly preferred
