Nurse Practitioner (NP) - San Jose, CA
Location: San Jose, CA
Job Type: Full-Time (Part-Time Considered)
Setting: 100% Outpatient | Onsite
Schedule: Monday-Friday, 8:30 AM - 5:00 PM | No Weekends | No On-Call
Position Overview
We are seeking a compassionate and dedicated Nurse Practitioner (NP) to join our outpatient primary care practice in San Jose, CA. This role focuses on delivering comprehensive primary and preventive healthcare services to a diverse patient population while collaborating with physicians and other healthcare professionals.
Key Responsibilities
Perform comprehensive patient evaluations, physical exams, and health assessments.
Diagnose and manage acute and chronic medical conditions.
Develop individualized treatment and preventive care plans.
Order, interpret, and review laboratory and diagnostic tests.
Prescribe medications and treatments in accordance with California regulations.
Provide preventive care, wellness counseling, and patient education.
Maintain accurate and timely electronic health records (EHR).
Deliver culturally sensitive, patient-centered care.
Qualifications Required
Master's or Doctorate from an accredited Nurse Practitioner program.
Active California Nurse Practitioner license.
Bilingual candidates who speak Korean, Tagalog, or Vietnamese (one or more) will be fast-tracked for an interview.
Active DEA registration.
New graduates with NP license are welcome to apply.
Candidates within 4-5 years of completing their NP program are preferred.
Advanced training or experience in cardiovascular care is highly preferred.
Experience using Electronic Health Record (EHR) systems.
