Position Highlights
Provide comprehensive primary care exclusively for seniors (65+ Medicare/Medicare Advantage)

Candidate Requirements
Licensed APRN in Florida and graduate of an accredited nursing program
1-3 years of primary senior care experience preferred
Confident managing chronic conditions such as diabetes, COPD, CHF, and hypertension
Proficient in EMR documentation with strong organizational skills
A natural communicator with genuine compassion and strong follow-through, bilingual in Spanish is preferred but not required
Passionate about senior-focused, value-based care and patient advocacy
