Avir Health Group is seeking a qualified Nurse Practitioner (Skilled Nursing) to join our team in a SNF setting, serving as clinical leader and collaborating with the facility care team to oversee residents.
Responsibilities include admissions, discharges, monthly stability checks, evaluations for changes in condition, advanced care planning, comprehensive assessments, and prescribing treatments. Master’s in Nursing and TX APN licensure required.
