Los Cerros Medical LLC El Paso, Texas

Los Cerros Medical LLC is seeking a dependable, compassionate, and clinically strong Nurse Practitioner to join our growing mobile primary care team in El Paso, Texas.

Los Cerros Medical provides comprehensive primary care directly in the homes of patients who may have difficulty accessing traditional office-based healthcare. Our patient population consists primarily of older adults, homebound individuals, and patients with multiple chronic medical conditions. Our goal is to provide high-quality, personalized healthcare while helping our patients remain safely and comfortably in their homes.

Position Overview

The Nurse Practitioner will provide in-home primary care services to an established panel of patients throughout the El Paso area. The ideal candidate is comfortable practicing with a high degree of autonomy, managing medically complex patients, recognizing changes in clinical condition, and coordinating care with other members of the healthcare team.

This position is an excellent opportunity for an NP who enjoys developing long-term relationships with patients and prefers a more personalized approach to healthcare than a traditional high-volume clinic environment.

Responsibilities

Perform comprehensive and problem-focused evaluations in patients' homes.

Diagnose and manage acute and chronic medical conditions.

Develop, implement, and update individualized treatment plans.

Manage common chronic conditions including hypertension, diabetes, COPD, CHF, hyperlipidemia, thyroid disorders, and other conditions commonly encountered in adult and geriatric primary care.

Review and interpret laboratory results, diagnostic studies, imaging reports, and other clinical information.

Order appropriate laboratory testing, imaging, medications, referrals, and additional services.

Perform medication reconciliation and ongoing medication management.

Recognize changes in patient condition and determine the appropriate level of intervention or escalation of care.

Provide preventive care, patient education, and chronic disease management.

Coordinate care with physicians, specialists, home health agencies, pharmacies, therapists, caregivers, and other members of the interdisciplinary team.

Complete accurate, thorough, and timely electronic medical record documentation.

Appropriately document medical necessity and clinical decision-making.

Maintain professional and compassionate communication with patients, families, caregivers, and staff.

Follow Los Cerros Medical clinical protocols, documentation standards, and quality expectations.

Qualifications

Current and unrestricted Texas Nurse Practitioner/APRN license .

Current national board certification as a Nurse Practitioner.

Graduate of an accredited Nurse Practitioner program.

Current prescriptive authority and ability to meet applicable credentialing requirements.

Strong assessment, diagnostic, and clinical decision-making skills.

Comfortable independently evaluating and managing medically complex adult and geriatric patients.

Proficient with electronic health records and computer-based clinical systems.

Strong written and verbal communication skills.

Reliable transportation and willingness to travel locally to patients' homes.

Dependable, organized, professional, and able to manage a mobile patient schedule effectively.

Primary care, internal medicine, geriatrics, home-based primary care, hospital medicine, or other experience managing medically complex adults is strongly preferred.

The successful candidate should be comfortable working independently while also understanding that excellent home-based medicine requires strong interdisciplinary teamwork.

We particularly value providers who are willing to continue learning and who can thoughtfully interpret clinical information rather than simply completing assigned visits.
