Job description
Nurse Practitioners - Nursing Home
Located in Philadelphia County, PA, Montgomery County, PA, Delaware County, PA, Lancaster County, PA, Lycoming County, PA, and Schuylkill County, PA
Salary: Up to $125K; Based on experience
Qualifications of the Nurse Practitioner:
Active Nurse Practitioner license in the state of Pennsylvania (required).
Experience working in long-term care, subacute rehab, or post-acute environments strongly preferred.
Strong communication and clinical decision-making skills.
Ability to work independently across contracted facilities.
Responsibilities of the Nurse Practitioner:
Provide clinical assessments, ongoing medical management, and patient-centered care in skilled nursing and long-term care settings.
Collaborate with physicians and interdisciplinary teams to support rehab goals and continuity of care.
Document patient encounters in accordance with regulatory and company standards.
Support quality initiatives and contribute to a consistent level of excellence across assigned facilities.
