Job Description Job Description The Nurse RN/LPN provides skilled nursing care to geriatric patients in a long-term care and skilled care facility. Reporting to the Director of Nursing, this role supports day and night shift coverage while delivering safe, compassionate, and coordinated patient care.
Responsibilities
Assess geriatric patients and monitor changes in condition.
Develop and implement individualized care plans.
Administer medications safely and accurately.
Monitor and document vital signs.
Provide wound care and related treatments.
Maintain accurate clinical documentation.
Educate patients on care needs and treatment plans.
Coordinate care, support infection control, and assist with discharge planning.
Required Qualifications
Active Registered Nurse (RN) license.
Active Licensed Practical Nurse (LPN) license.
