Description:
Provide skilled care to residents.
Assist residents with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
Provide mobility support, including transfers and ambulation.
Assist with feeding residents and documenting intake/output.
Ensure residents' comfort by maintaining clean and safe living environments.
Provide perineal care and uphold resident hygiene.
Monitor residents' physical and emotional status and report any changes to nursing staff.
Measure and record vital signs as directed.
Observe infection control protocols diligently.
Document all care provided in daily logs or electronic medical records.
Communicate resident needs and changes in condition to nursing staff.
Participate in shift hand-off reports for continuity of care.
Maintain a safe environment for residents and prevent falls.
Apply safety devices as directed by nursing staff.
Follow protocols for handling emergencies.
Provide emotional support and reassurance to residents.
Encourage independence while assisting with care.
Respect and uphold residents' rights, dignity, and privacy.
Assist residents in participating in recreational and social activities.
Work under the supervision of an RN or LPN, following directions carefully.
