Job description
Hartford Healthcare seeks an OASIS Registered Nurse – HomeCare to deliver skilled nursing care to patients in their homes. The RN completes comprehensive OASIS assessments, develops individualized care plans, and provides treatments such as wound care, medication administration, and chronic disease management. This role coordinates with physicians, therapists, and caregivers to ensure quality, patient-centered care.
