Hartford Healthcare seeks an experienced OASIS RN for weekend home health visits. In this field-based role, you will complete comprehensive OASIS assessments, develop individualized care plans, administer treatments, and educate patients and families in their homes. You will document accurately in the EHR, monitor changes in condition, and collaborate with an interdisciplinary team.
