Job description
Hartford HealthCare at Home is the largest provider of homecare services in Connecticut, seeking an OASIS RN to deliver high-quality, outcome-based nursing in patients' homes. The role focuses on completing OASIS assessments, developing care plans, and coordinating with a multidisciplinary team.
The position includes leading care teams, supervising LPNs and Home Care Aides, performing timely visits, educating clients and families, and engaging in continuous improvement initiatives.
