Job description
Hartford Healthcare seeks a Weekend OASIS RN to deliver high-quality home-based care across our Connecticut network. In this role, you’ll perform comprehensive OASIS assessments, develop and update care plans, administer medications and treatments, and educate patients and families to support safe, effective care at home. You’ll collaborate closely with an interdisciplinary team, utilizing advanced clinical tools and EHR systems.
