OCCUPATIONAL THERAPIST
I. General description:
To provide occupational therapy to identified students. Supervises: Certified Occupational Therapy Aide/Assistant (C.O.T.A.) (if applicable)
II. Qualifications:
Current American Occupational Therapy Certification
Board or National Board for Certification in Occupational Therapy, Inc. certification and Current licensure from the Arizona Board of Occupational Therapy Examiners
Must possess a Valid AZ Department of Public Safety fingerprint clearance card.
Must be able to pass a background check upon employment
III. Duties and Responsibilities:
Screen and evaluate appropriately referred students within the State and Federal requirements.
Attend and actively participate in all appropriate Child Study Team (C.S.T.), Section 504, Multidisciplinary Evaluation Team (M.E.T.), and Individualized Education Program (I.E.P.) meetings.
Develop occupational therapy related IEP/504 goals, objectives, and/or benchmarks, including any appropriate accommodations for qualified district students as assigned.
Complete all State and Federally required paperwork within required timelines.
Provide individual and group occupational therapy services to students as described in the IEP and 504 Plans.
Prepare quarterly, or more frequent if needed, progress reports on each assigned student to include in the quarterly grade reporting process.
Complete all MEDICAD reports and requirements in a timely manner.
Participate in District training as appropriate.
Perform other related duties as may be assigned by the Director of Special Services.

The employee must occasionally lift and/or move up to 25 pounds. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
