Required Qualifications
Occupational Therapist, Acute Experience: 1 year Licenses: OT-AZ(Copy needed) Certifications: BLS-AHA(Copy Needed)
OTR-NBCOT(Copy Needed) SSN Required DOB Required Work History: Should not be more than 90 days of gap in last 1 year
