nFamAli LLC is a Qualified Vendor contracted with Arizona’s Division of Developmental Disabilities (DDD). We provide person-centered therapy and support services to individuals with developmental disabilities across Arizona. Our mission is to promote independence, inclusion, and quality of life through evidence-based and culturally responsive care.
nAs an Occupational Therapist, you will deliver services in alignment with AHCCCS and DDD standards, collaborating with interdisciplinary teams, families, and DDD Support Coordinators.
nKey Responsibilities
nConduct DDD-compliant occupational therapy evaluations, including functional, sensory-motor, fine motor, self-care, and adaptive equipment assessments.
nDevelop and implement Individualized Treatment Plans (ITPs) based on member needs, ISP goals, and regulatory guidelines.
nProvide direct OT services in home, community, or facility-based settings.
nRecommend and train on assistive technology, adaptive devices, and environmental modifications.
nParticipate in Individual Support Plan (ISP) meetings and provide therapy recommendations to the team.
nEducate and consult with caregivers, families, and direct support professionals on therapeutic strategies and carryover activities.
nMaintain accurate and timely documentation in accordance with DDD, AHCCCS, and Medicaid requirements.
nMonitor progress, revise goals, and support discharge planning as appropriate.
nStay current with Arizona OT licensure, DDD policies, and best practices in developmental disability services.
nComply with AHCCCS Policy 310-X regarding scope, frequency, and medical necessity of OT services.
nQualifications & Requirements
nMaster’s or Doctoral degree in Occupational Therapy from an accredited program
nCurrent Arizona OT license (required)
nNBCOT certification (preferred)
nExperience with developmental disabilities, pediatric populations, or DDD services (preferred)
nStrong clinical reasoning, assessment, and documentation skills
nProficiency in electronic health records and therapy documentation systems
nValid Arizona Fingerprint Clearance Card
nReliable transportation and valid driver’s license for community-based visits
nAbility to meet DDD provider credentialing and compliance standards
