nChautauqua Nursing & Rehabilitation Center is looking to add an Occupational Therapist (OT) to its rehabilitation team. In this role, you'll evaluate residents, develop individualized treatment plans, and provide therapy services that help residents regain independence, improve daily living skills, and achieve their highest level of function. nRESPONSIBILITIES:
nnnEvaluate residents and develop individualized occupational therapy treatment plans
n
nnProvide skilled therapy services focused on improving functional abilities, ADLs, mobility, and quality of life
n
nnEstablish short- and long-term rehabilitation goals and monitor resident progress
n
nnCollaborate with physicians, nursing staff, families, and interdisciplinary team members to support resident outcomes
n
nnSupervise Certified Occupational Therapy Assistants (COTAs) in accordance with state regulations
n
nnComplete all required documentation, assessments, care plans, and billing records accurately and timely
n
nnProvide resident and family education regarding therapy goals, adaptive equipment, and home exercise programs
n
nnRecommend and coordinate adaptive equipment and durable medical equipment (DME) needs
n
nnParticipate in care conferences, discharge planning, staff education, and quality improvement initiatives
n
nnPerform additional duties as assigned

n nREQUIREMENTS:
nnnMaster's Degree in Occupational Therapy from an accredited program approved by the American Occupational Therapy Association
n
nnValid New York State Occupational Therapist License required
n
nnNewly licensed Occupational Therapists encouraged to apply
n
nnSkilled nursing facility experience preferred
n
nnStrong clinical, documentation, communication, and problem-solving skills
n
nnAbility to work collaboratively within an interdisciplinary healthcare team
n
nnCommitment to resident-centered care and rehabilitation excellence
