Wellsprings of Gilbert, a premier short-term rehabilitation and skilled nursing facility in Gilbert, Arizona, is seeking a compassionate and motivated Occupational Therapist to join our outstanding in-house therapy team. We have Full-Time, Part-Time, and PRN opportunities available for clinicians who are passionate about helping patients regain independence and return home safely.

Compensation
$47 - $53 per hour based on experience

Qualifications
Graduate of an accredited Occupational Therapy program
Current Occupational Therapist license in Arizona (or ability to obtain)
New graduates and experienced therapists encouraged to apply
Essential Job Duties
Evaluate patients with cognitive, physical, and ADL deficits and develop individualized treatment plans
Utilize standardized assessments and evidence-based interventions
Provide skilled occupational therapy services in accordance with physician orders and regulatory requirements
Implement therapeutic exercises, activities, ADL retraining, adaptive equipment training, and other treatment interventions
Monitor patient progress and modify treatment plans as appropriate
Collaborate with physicians, nursing staff, families, and interdisciplinary team members
Educate patients, caregivers, and families regarding treatment plans and functional outcomes
Complete timely and accurate documentation
Prepare comprehensive discharge recommendations and summaries
