OCCUPATIONAL THERAPIST (OT)

About the Facility
Ahava Therapy Services is hiring a Full-Time Occupational Therapist for Henderson Health & Rehab , a 137-bed skilled nursing facility that provides short- and long-term care and outpatient services. Therapists at Henderson work as part of an in-house collaborative rehab team, with strong leadership support, to help residents achieve independence and optimal function.

It is known for its affordable living, Freed-Hardeman University, quaint downtown, and proximity to attractions like Pinson Mounds and Chickasaw State Park, offering a sparse suburban feel with highly rated public schools.
Responsibilities
Review clinical information and evaluate its impact on the assessment and treatment process.
Conduct comprehensive functional assessments using standardized procedures.
Develop and implement individualized treatment plans with achievable functional goals.
Document evaluations, daily treatments, progress notes, and discharge summaries accurately and promptly.
Revise treatment plans based on patient performance and progress.
Supervise and guide therapy assistants, ensuring proper treatment techniques and the delivery of care.
Educate residents, families, and caregivers about exercises and therapeutic procedures to support carryover.
Collaborate with nursing staff, interdisciplinary team members, and families to maximize functional outcomes.
Assist with discharge planning and maintain knowledge of community resources.
Participate in care conferences, meetings, and quality improvement initiatives.
Maintain clinical competence through continuing education opportunities.
Adhere to all regulatory, professional, and facility standards, including HIPAA and payer guidelines.
Support the company's mission, vision, and professional expectations.
Maintain expected productivity levels and demonstrate excellent teamwork and communication skills.
Qualifications
Bachelor's or Master's Degree in Occupational Therapy
Active OT license in the state of practice
BLS/CPR certification (current or obtained during orientation)
Strong clinical reasoning, communication, and documentation skills
Ability to collaborate effectively within an interdisciplinary team
