Job description
Archbold Home Health Services, part of the Optum family, seeks an Occupational Therapist to assess and address patients’ functional status, ADLs, and home safety needs within the home health setting.
You will evaluate, document, and coordinate therapy plans with the care team, ensuring evidence-based practices and CMS compliance while supporting patient independence and safety at home.
