Job Summary and Responsibilities
nThe primary service area for this position will be Leadville, Fairplay & Frisco
nCommonSpirit Health at Home leverages AI-powered clinical documentation to reduce documentation, enhance work-life balance, and maximize patient time.
nWe are seeking a skilled and compassionate Occupational Therapis t to join CommonSpirit Health at Home. The OT will provide skilled therapy services to patients in their home evaluating the patients abilities and needs, setting goals and developing a plan to help the patient achieve independence, improve their quality of life and prevent potential health-related problems.
nResponsibilities:
nnnAssess patients' physical, cognitive, and psychosocial abilities to determine therapy needs.Develop and implement personalized treatment plans that may include exercises', adaptive techniques, and use of assistive devices to improve patients functional abilities.

nnTeach patients and family members about home safety, adaptive equipment, and strategies to manage daily activities effectively.
n
nnMaintain accurate and timely records of patient progress, treatment plans, and modifications to therapy programs for compliance and regulatory standards.
n
nnMonitor patients progress and adjust treatment plans as necessary to meet changing needs. Utilize outcome-based measures to assess the effectiveness of therapy interventions.

nnCollaborate and communicate patient's change in condition with physicians, nurses, physical therapists, speech therapists, and other home health team members.

n nJob Requirements
nnnBachelor's degree from an accredited school approved by the American Occupational Therapy Association
n
nnActive registered OT license in the state which you practice
n
nnMust obtain and maintain current CPR/BLS Certification
n
nnOne-year Occupational Therapy experience in a clinical setting.
n
nnCurrent Drivers License
n
nnComply with federal, state and local laws and regulations

n$39.18 - $64.65 /hour
