nBradenton Area, FL | Mobile Outpatient Therapy Part-Time to Full-Time
nFunctional Transformation Mobile Rehab is seeking a compassionate and motivated Occupational Therapist specializing in Lymphedema Therapy to join our growing therapist-owned practice serving the Bradenton and surrounding areas.
nWe specialize in bringing high-quality, patient-centered lymphedema care directly to patients in their homes. Our therapists have the opportunity to provide meaningful, one-on-one care without the unrealistic productivity expectations often found in traditional healthcare settings.

nQualifications
nActive Florida Occupational Therapy license
nCertified Lymphedema Therapist (CLT) preferred; willingness to obtain certification may be considered
nStrong understanding of lymphedema treatment and current therapy practices
nAbility to work independently in a mobile setting while remaining connected with the clinical and administrative team
nExcellent interpersonal and communication skills
nProfessional, dependable, and compassionate
nComfortable working with patients with a wide range of medical needs, personalities, and home environments
nCommitment to continued learning and professional growth
nResponsibilities
nPerform comprehensive occupational therapy and lymphedema evaluations
nDevelop and implement individualized plans of care based on each patient's needs and goals
nProvide skilled lymphedema therapy, including appropriate components of Complete Decongestive Therapy (CDT)
nAssess patients' ability to safely perform activities of daily living
nEducate patients, family members, and caregivers regarding treatment, self-management, and home programs
nDocument evaluations, treatments, progress, and outcomes accurately and in a timely manner
nMonitor progress toward short- and long-term goals and adjust treatment plans as appropriate
nAssist with discharge planning and recommendations for adaptive equipment when needed
nCommunicate with physicians, caregivers, and other members of the patient's healthcare team as appropriate
nCollect and document required treatment information and follow up with patients as needed
