Position Overview
We are seeking a qualified candidate with expertise in skilled nursing facilities (SNF) to join our team. The ideal candidate will have a minimum of one year of experience in an SNF environment, hold an active California license, and possess knowledge of NetHealth (Rehab Optima).
Key Responsibilities
Provide high-quality care to residents in a skilled nursing facility.
Utilize electronic health record systems, specifically NetHealth (Rehab Optima), for patient documentation and management.
Collaborate with a multidisciplinary team to assess and coordinate resident care.
Maintain compliance with state regulations and facility policies.
Qualifications Minimum of one year of experience in a skilled nursing facility.
Active California nursing license.
Proficiency in NetHealth (Rehab Optima) software.
Excellent communication and interpersonal skills.
Strong organizational skills and attention to detail.
