Job description
Home Health of Western Colorado is seeking a compassionate Occupational Therapist to provide in-home therapy services across Western Colorado. This role evaluates functional abilities, develops individualized treatment plans, and delivers hands-on interventions to help patients regain independence in daily activities. The therapist will educate patients and families, recommend adaptive equipment, and coordinate care with an interdisciplinary team. Ideal candidates demonstrate strong clinical judgment, documentation skills, and a commitment to patient-centered, community-focused care in a home health setting.
