Job description
Occupational Therapist (OT) – Home Health – Per Visit / PRN
Penn Home Health Care is seeking a motivated Occupational Therapist (OT) to provide patient-centered care in the home setting.
Service Areas: Philadelphia, Delaware, Chester, Montgomery, Bucks, and Berks Counties, PA
Position Type: Per Visit / PRN
Responsibilities:
Complete evaluations and reassessments.
Develop treatment plans focused on activities of daily living and functional independence.
Provide therapeutic interventions and adaptive equipment recommendations.
Educate patients and caregivers.
Maintain accurate and timely documentation.
Qualifications:
Current Pennsylvania Occupational Therapist license.
Current CPR certification.
Valid driver's license and reliable transportation.
Home health experience preferred.
