nPhoenix Home Care and Hospice is hiring an Occupational Therapist to support patients in the Jefferson City and Columbia areas. This is your chance to move beyond the clinic setting and help patients regain independence in their own homes.

nWhat You Will Do
nnnProvide occupational therapy services in the home based on the physician plan of care
n
nnEvaluate patient needs and develop individualized treatment plans
n
nnFocus on improving daily living skills, safety, and independence
n
nnCollaborate with the care team to ensure quality outcomes
n
nnEducate patients and families on techniques and care plans
n
nnMaintain accurate and timely documentation
n
nnFollow all safety and compliance standards
n
n nWhat You Need
nnnActive Occupational Therapist license in Missouri
n
nnHome health experience preferred but not required
n
nnStrong clinical judgment and time management skills
n
nnAbility to work independently and manage your schedule
n
nnStrong communication and patient education skills
n
nnAbility to pass background check, drug screen, and physical
n
