Occupational Therapist (OT) - Post-Acute Rehab | Skilled Nursing Facility (SNF) | PRN
Concept Rehab is hiring an Occupational Therapist (OT) to join our therapy team in Johnston, Iowa.

Requirements
About the Role
As an Occupational Therapist (OT), you will evaluate and treat patients recovering from illness, injury, or surgery in a skilled nursing and post-acute rehabilitation environment. You'll develop individualized treatment plans that improve function, mobility, safety, cognition, and independence while collaborating with patients, families, physicians, nursing staff, and fellow rehabilitation professionals.
This position is ideal for Occupational Therapists (OTs/OTRs) who enjoy variety, flexibility, and making a meaningful difference in patients' lives.
Responsibilities Evaluate patients and develop individualized treatment plans.
Provide evidence-based occupational therapy services.
Supervise Occupational Therapy Assistants (COTAs) as appropriate.
Collaborate with physicians, nursing staff, rehabilitation professionals, patients, and families.
Participate in interdisciplinary care planning.
Educate patients, caregivers, and facility staff.
Complete timely documentation and billing.
Recommend adaptive equipment and discharge planning needs.
Maintain compliance with state licensure requirements and company policies.
Demonstrate Concept Rehab's values of Commitment, Respect, and Integrity.
Qualifications Graduate of an accredited Occupational Therapy program.
Current Occupational Therapist (OT) license in the state of practice (or eligible for licensure).
Excellent communication and teamwork skills.
Passion for helping older adults maximize function and independence.
