Human Touch Home Health is seeking a compassionate and skilled Occupational Therapist (OT) to join our growing team serving patients throughout Prince George's County and Montgomery County, Maryland , including Hyattsville, Brentwood, Mount Rainier, Bowie, Greenbelt, Lanham, New Carrollton, Largo, Upper Marlboro, Clinton, Bethesda, Silver Spring, Rockville, Gaithersburg, Germantown, Wheaton, Olney, Kensington, Potomac, Damascus, Derwood, and surrounding communities.

As a Home Health Occupational Therapist, you'll help patients regain the skills, strength, and confidence they need to safely perform everyday activities while remaining in the comfort of their own homes.
You'll have the freedom to create individualized treatment plans, build meaningful relationships, and celebrate every milestone alongside your patients.

What You'll Do Complete occupational therapy evaluations, reassessments, Start of Care (SOC), and discharge visits
Develop individualized treatment plans based on patient goals and functional needs
Provide skilled occupational therapy services in patients' homes
Assess home environments and recommend adaptive equipment and safety modifications
Educate patients and caregivers on therapeutic techniques, home safety, energy conservation, adaptive equipment, and activities of daily living
Monitor patient progress and modify treatment plans as appropriate
Collaborate with physicians, nurses, social workers, and the interdisciplinary care team
Complete timely and accurate clinical documentation using WellSky
Qualifications Active Maryland Occupational Therapist License Required
Valid Driver's License and Reliable Transportation
Strong Clinical Assessment, Communication, and Documentation Skills
Home Health Experience Preferred but Not Required
OASIS Experience Preferred
Experience with WellSky Home Health is a plus
