nPosition Purpose:
nJoin Trinity Health At Home as a Home Care Occupational Therapist ( OT ) and make a meaningful impact by delivering compassionate, high-quality therapy services to clients in their homes. You’ll help individuals regain independence and improve their quality of life through personalized care plans and therapeutic interventions.
nCoverage Regions:
nRomulus, Bellville, Van Buren, Whittaker, Willis, and New Boston
nDearborn, Dearborn Heights, Garden City, Livonia, Taylor, Wayne, Westland, Redford
nOakland County: Waterford & Pontiac
nLivingston County: Brighton, Chelsea, Dexter, Howell
nWhat You Will Do:
nnnAssess clients’ functional status and develop individualized treatment plans in collaboration with physicians.
n
nnDeliver preventive, therapeutic, and rehabilitative services in accordance with the plan of care.
n
nnEducate clients and families on safety, health promotion, and use of prosthetic/orthotic devices.
n
nnSupervise Certified Occupational Therapy Assistants (COTAs) and Home Health Aides (HHAs).
n
nnCoordinate care with interdisciplinary teams and participate in case conferences.
n
nnDocument all care activities accurately using point-of-care systems.
n
n nMinimum Qualifications:
nnnGraduate of an accredited Occupational Therapy program.
n
nnNBCOT certification and state licensure/registration.
n
nnMinimum one year of experience or completion of a 6–8 week preceptorship.
n
nnValid driver’s license and reliable transportation.
n
nnHome health experience preferred.
n
nnCommitment to ethical standards and continuous learning.
