The Chateau at Brooklyn Rehabilitation & Nursing Center in Brooklyn, NY is 189-bed rehabilitation center offering comprehensive short- and long-term rehabilitation programs, including PT, OT, ST, and respiratory services, a spacious rehabilitation gym, free on-site parking, and convenient access to public transportation, with the B36 bus stopping right in front and the B43 and B3 buses just one block away.. Hiring for this role is done on site by our Director of Rehab and facility leadership, not by an outside recruiter. You will interview with the people you will actually work with.

We are part of a growing network of 35 skilled nursing facilities, which means real clinical support behind you and real places to go from here.

Why This OT Role Is Different

Interdisciplinary by design. OT, PT, SLP, nursing team, dietitian, respiratory, social work, and MDS work from the same plan and the same table. You will not be treating in a silo.

A training environment. New graduates are welcome. You get structured mentorship, hands on orientation, and ongoing clinical education from clinicians who want to teach.

Leadership that is present. Our Director of Rehab and facility administration are invested in where you want to be in five years and will talk with you about it regularly, not only once a year at review time.

Growth you can actually reach. A 35 facility network that is still expanding means specialty program development, mentorship roles, and leadership tracks, including:

Dementia clinical focus champions

Respiratory champions and step down

Adaptive equipment liaison specialists

Assistant director and lead or senior therapist ladder programs

Work life balance is an operating priority. Tell us the schedule you need, and we will work with you to build it. Our residents come first, and happy therapists equal happy patients.

What You Will Do

Evaluate referred residents to determine medical status, functional ability, limitations, precautions, and rehabilitation potential

Build individualized treatment plans with measurable, achievable goals based on each resident’s capacity and tolerance, under physician direction

Deliver skilled occupational therapy interventions focused on ADL performance, safety, and return to prior level of function

Complete evaluations, weekly progress notes, recertifications, and discharge documentation accurately and on time

Educate residents, families, and caregivers on therapy techniques, safety, and carryover strategies

Collaborate with PT, SLP, nursing, dietitians, and social work to deliver patient centered care and support discharge readiness

Recommend adaptive equipment, durable medical equipment, and environmental modifications

Supervise Occupational Therapy Assistants and students in accordance with the state practice act

What You Will Need

Degree in Occupational Therapy from an accredited program

Valid Occupational Therapist license and registration in the state of practice, or license eligible

Ability to supervise Occupational Therapy Assistants in accordance with the state practice act

Comfort with clinical documentation including evaluations, weekly progress notes, recertifications, and discharge summaries

Commitment to confidentiality, HIPAA, and Federal Resident Rights

Preferred, Not Required

Experience in long term care, sub acute, or geriatric rehabilitation

Prior experience in an interdisciplinary care model

New graduates and therapists new to skilled nursing are encouraged to apply. We train.
