n*Evaluates and re-evaluates referred patients ability to perform functional skills and

nplans a treatment program designed to meet identified goals

n*Initiates communication with physicians and other professionals to ensure

ncoordination of treatment efforts. Contacts family members and outside agents.

nMaintains ongoing communication with immediate Supervisor, subordinates, and peers

n*Performs other job-related duties within job scope as requested by Supervisor

nMinimum Qualifications :

nnGraduate of Accredited School of Occupational Therapy is required.

n nLicensure:

nnCurrent National Occupational Therapy Certification/ Registration and Licensed

n nOccupational Therapist in the State of Florida is required.
