nAs an Occupational Therapist at CenterWell Home Health, you'll help patients regain strength, mobility and independence-all from the comfort of their homes. By delivering personalized care that focuses on rehabilitation and functional improvement, you'll empower individuals to overcome physical limitations, perform everyday activities with confidence and enjoy a better quality of life. You will report the Clinical Manager.

nResponsibilities:

nnnAssess/screen patient's daily living/work-related skills and develop therapeutic retraining programs with measurable goals. Administer and interpret diagnostic and prognostic tests of function to patients in their homes or other settings to assist the physician in evaluating the patient's level of function.

nnConfer with the patient's physician and other health care team members and participate in development/revision of the Plan of Care Treatment.

n

nnProvide therapeutic treatment and instruction to patients following physician orders to improve/restore strength, coordination, range-of-motion and function or teach compensation measures.

n

nnReview/expand the retraining programs in a manner consistent with the behavioral goals of each patient and within the guidelines of the rehabilitation program as a whole.

n

nnConsult with other vocational team members to develop and implement vocationally oriented plans consistent with the needs and capabilities of patients.

nnReport patient status and progress to the physician, rehabilitation staff, Clinical Manager, patient and family members.

n

nnParticipate in care coordination and discharge planning activities and act as a resource to other health care personnel in meeting patient's needs.

n

nnDesign community reintegration activities to assist the client in the physical reconditioning effort, and the psychological adjustment and coordinate the plan with members of the interdisciplinary team.

n

nnRecommend and design special adaptive equipment for clients to improve residence or working environments or improve their participation in the rehabilitation program and community.

n

nnDesign/train staff and family members to manage the retraining program including dressing, feeding, grooming and hygiene skills, participate in team and family educational meetings.

n

nnOversee and instruct and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.

n

nnAccurately and thoroughly document patients' care observations, interventions and evaluation on the day services are rendered.

n

nnEnsure that interim (verbal) orders received from the physician are accurately documented and implemented.

n

nnSubmit evaluation, treatment plans and discharge summary to the supervisor and care management staff.

n nUse your skills to make an impact

nRequired Experience/Skills:

nnnDegree from an accredited Occupational Therapy Program

n

nnA minimum of six months of occupational therapy experience preferred

n

nnHome Health experience a plus

n

nnCurrent and unrestricted OT licensure

n

nnCurrent CPR certification

n

nnGood organizational and communication skills

n

nnA valid driver's license, auto insurance, and reliable transportation are required.

n

n nPay Range

nnn$54.00 - $76.00 pay per visit/unit

n

nn$85,400 - $117,500 per year base pay

n

nThe compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.
